People asking how long bacteriostatic water lasts usually mean one of three things: how long an unopened vial keeps, how long an opened vial can be entered, or how long a peptide mixed with it stays good. Those are three different questions with three different answers, and forum shorthand tends to collapse them into one number.

Short answer: unopened, until the printed expiry at room temperature. Opened, standard practice is to date the vial and discard within 28 days unless the manufacturer says otherwise. A reconstituted peptide gets no shelf life from the water at all; it follows its own product data, which research-market vials usually do not have.

Unopened: the printed expiry and room-temperature storage

Bacteriostatic Water for Injection, USP is a sterile, nonpyrogenic water for injection with 0.9% (9 mg/mL) or 1.1% (11 mg/mL) benzyl alcohol added as a preservative. Hospira supplies it in a multiple-dose 30 mL plastic fliptop vial and a 20 mL fill in a 30 mL glass vial. The label directs storage at 20 to 25°C (68 to 77°F), which is USP controlled room temperature.

Three practical consequences:

  1. Refrigeration is not required. Many people refrigerate the diluent because they refrigerate reconstituted peptides. The diluent label does not ask for it.
  2. Freezing should be avoided. Freezing can compromise the container seal and the solution. A vial that has been frozen should be treated as suspect.
  3. The printed date is the limit. An unopened, sealed vial past its expiry is not "probably fine" because it looks clear. Clarity says nothing about preservative potency or container integrity over time.

Storage before opening is the easy part. The label answers it completely.

Opened: where the 28-day rule actually comes from

The 28-day figure that circulates on every peptide forum is not a stability result and it is not printed on the water's label. It is a healthcare infection-control practice for multi-dose vials.

CDC's injection-safety guidance states that once a multi-dose vial is opened or needle-punctured, it should be dated and discarded within 28 days unless the manufacturer states another date. The same guidance asks clinicians to assign multi-dose vials to a single patient whenever possible, to keep them out of the immediate treatment area, and to use a new sterile needle and syringe for every entry.

The rule exists because a preserved vial accumulates risk with each puncture. Benzyl alcohol is bacteriostatic, meaning it inhibits bacterial growth. It does not sterilize a contaminated needle, it does not act on every organism, and it does not reverse poor technique. The 28-day window is a conservative default that keeps cumulative contamination risk inside a margin that infection-control bodies are comfortable with.

Why clinical settings take the rule seriously

Injection-safety outbreaks are the reason the guidance exists. Published investigations have traced hepatitis C transmission to shared vials and syringes, and infection-control reviews consistently recommend single-dose vials where possible and strict discard practices for multi-dose containers. A home user is not in a clinical setting, but the biology of a punctured vial is the same.

After mixing: the diluent gives the peptide no shelf life

This is the row most often misread. Bacteriostatic water makes the diluent vial suitable for repeated entry. It does not transfer a 28-day, 30-day or any other period onto a reconstituted peptide.

The Hospira label is explicit: reconstituted drug solutions should not be stored unless the manufacturer of the solute directs otherwise. In other words, once the water goes into a peptide vial, the peptide's label owns the storage question. Approved products answer it. EGRIFTA WR, a tesamorelin kit, ships with its own supplied diluent and a product-specific discard period after mixing. Approved GLP-1 pens are ready-to-use and involve no reconstitution.

Research-market vials have a certificate of analysis showing purity at manufacture. They do not have a reconstituted-stability study, a preservative-compatibility study, or a container-closure study. So a claim that "bacteriostatic water keeps it good for 28 days" borrows a diluent handling rule and applies it to a product that has no data. There is also a chemistry reason to be cautious. A published formulation study found that benzyl alcohol promoted aggregation of a reconstituted lyophilized protein, with the effect depending on concentration and temperature. Compatibility is product-specific. Some drugs are compatible with preserved diluent, some are not, and the diluent label itself warns that some drugs may be incompatible in a given vehicle.

Signs a vial should be discarded early

The 28-day window is the outer limit, not a promise. Discard sooner if:

  • The solution is cloudy, discolored, or contains visible particles.
  • The stopper or seal is damaged, or the vial was dropped and cracked.
  • The vial was left uncapped, entered with a used needle, or handled in a way that breached aseptic technique.
  • The vial was not dated on first puncture and its opening date is unknown. If you cannot prove it is inside the window, treat it as outside.
  • The vial was frozen or exposed to heat well above room temperature.

A clear solution does not prove sterility. Bacterial contamination is not reliably visible.

Bacteriostatic water vs sterile water: shelf life after opening

The comparison matters because people substitute one for the other without realizing the discard rules change.

Sterile water does not get a 28-day window because there is nothing in it to slow growth after the first puncture. Bacteriostatic saline is a different vehicle again, and drug labels specify which one to use.

Warnings that do not change with shelf life

Two label warnings apply on day one and on day 28 alike:

  • Not for neonates. Benzyl alcohol has been linked to a fatal toxicity syndrome in premature and low-birth-weight infants. Solutions containing it must not be used in that population.
  • Not for intravenous use without a solute. Water without solute is hypotonic, and the label warns that intravenous administration may cause hemolysis. Solutions must be made approximately isotonic before IV use.

Neither of these is a storage issue, but both get lost when the diluent is treated as "just water."
