FDA-Approved GLP-1 Drugs for Weight Loss in 2026
Every FDA-approved GLP-1 medication for weight loss in 2026: Wegovy, Zepbound, Saxenda, the newer oral Wegovy and Foundayo (orforglipron) — with approval dates and indications.

The FDA has now approved five GLP-1 medications specifically for chronic weight management — three injectable, two oral. Every other GLP-1 used for weight loss (Ozempic, Mounjaro) is being prescribed off-label — which is legal but doesn't qualify for weight-management-specific savings programs.
The official approved list for weight loss, as of 2026:
FDA-approved GLP-1 medications for weight management
| Drug (brand) | Active ingredient | Form | Approval (weight indication) |
|---|---|---|---|
| Saxenda | Liraglutide | Daily injection | 2014 |
| Wegovy | Semaglutide | Weekly injection | 2021 |
| Zepbound | Tirzepatide | Weekly injection | 2023 |
| Wegovy oral | Semaglutide | Daily oral pill | December 2025 |
| Foundayo | Orforglipron | Daily oral pill | 2026 |
The eligibility criteria on all five are the same:
- BMI ≥ 30 (obesity), OR
- BMI ≥ 27 with at least one weight-related condition (hypertension, dyslipidemia, type 2 diabetes, cardiovascular disease, obstructive sleep apnea, etc.)
Each drug has its own dosing, side-effect profile and average trial results.
Saxenda (liraglutide)
The first GLP-1 ever approved for weight management.
- Approved (weight): 2014
- Maker: Novo Nordisk
- Form: Daily subcutaneous injection
- Dose range: 0.6 → 3.0 mg/day (weekly titration)
- Trial weight loss: ~8% body weight at 56 weeks (SCALE trial)
- Cash-pay: ~$1,349/month list
Pros: Long track record; daily injection has a shorter half-life if you want to stop quickly. Cons: Daily dosing is an adherence burden; lower average weight loss than newer weekly options.
Wegovy (semaglutide)
The breakthrough that made GLP-1 weight loss mainstream.
- Approved (weight): 2021
- Maker: Novo Nordisk
- Form: Weekly subcutaneous injection
- Dose range: 0.25 → 2.4 mg/week, plus Wegovy HD 7.2 mg
- Trial weight loss: ~15% body weight at 68 weeks (STEP-1)
- Cash-pay: ~$1,349/month list; $349/month via NovoCare direct
- Additional approvals: Cardiovascular risk reduction (SELECT, 2024); MASH with moderate-to-advanced fibrosis (2024–2025)
Strongest cardiovascular evidence in the class. Now also approved for MASH (metabolic-associated steatohepatitis).
Zepbound (tirzepatide)
The current weight-loss leader by trial average.
- Approved (weight): 2023
- Maker: Eli Lilly
- Form: Weekly subcutaneous injection (pens, KwikPen, LillyDirect vials)
- Dose range: 2.5 → 15 mg/week
- Trial weight loss: ~21% body weight at 72 weeks (SURMOUNT-1)
- Cash-pay: ~$1,299/month (pens); $349–$499/month via LillyDirect vials
- Additional approvals: Moderate-to-severe obstructive sleep apnea in adults with obesity (2024)
The dual GLP-1 / GIP agonist mechanism is what makes tirzepatide outperform single-receptor semaglutide on average weight loss.
Wegovy oral (semaglutide pill)
Same active molecule as injectable Wegovy, in a pill.
- Approved: December 2025
- Maker: Novo Nordisk
- Form: Daily oral tablet
- Dose range: 0.25 → 25 mg/day (much higher than injection because of poor oral bioavailability)
- Trial weight loss: ~13% body weight at 64 weeks (OASIS program)
- Cash-pay: $149/month for 1.5 mg and 4 mg (intro pricing through Aug 2026, then $199); $199/month for 0.25 / 0.5 mg start doses
- How to take it: Empty stomach, first thing in the morning, 30 minutes before any food, drink (except water), or other medication
The empty-stomach requirement is a real adherence challenge for people already taking medications like levothyroxine or omeprazole.
Foundayo (orforglipron)
The first oral small-molecule GLP-1 — easier to take than oral Wegovy.
- Approved: 2026 (under FDA National Priority Voucher program)
- Maker: Eli Lilly
- Form: Daily oral tablet
- Dose range: 0.8 → 17.2 mg/day (six strengths)
- Trial weight loss: ~12% body weight at the highest dose; ~27 lb average vs ~2 lb placebo in 72-week trial
- Cash-pay: From $149/month via LillyDirect
- Insurance options: ~$25/month with commercial insurance + Lilly discount card; ~$50/month with Medicare Part D starting July 2026
Foundayo's edge: it can be taken any time of day, with or without food — unlike oral Wegovy. The trade-off is somewhat lower average weight loss than the injectables.
What's NOT FDA-approved for weight loss (but commonly used off-label)
These GLP-1 drugs are FDA-approved for type 2 diabetes, not weight loss. Doctors can prescribe them off-label for weight loss, and many do, but:
- They don't qualify for weight-loss-specific savings programs
- Insurance generally won't cover them for off-label weight loss
- The cash-pay price applies in full
The big off-label ones:
- Ozempic (semaglutide, weekly injection) — approved for T2D, not weight management
- Mounjaro (tirzepatide, weekly injection) — approved for T2D, not weight management
- Rybelsus (semaglutide, daily oral 3-14 mg) — approved for T2D
- Trulicity (dulaglutide, weekly injection) — approved for T2D
- Victoza (liraglutide, daily injection) — approved for T2D
- Byetta / Bydureon (exenatide) — approved for T2D

Pipeline: drugs heading for weight-management approval
- Retatrutide (Eli Lilly) — triple agonist GLP-1/GIP/glucagon. Phase 3 TRIUMPH program reading through 2026; NDA expected late 2026; weight-management approval projected 2027–2028. Phase 2 results showed ~24% weight loss. See our Retatrutide guide.
- Survodutide (Boehringer Ingelheim / Zealand) — dual GLP-1/glucagon agonist; Phase 3 for obesity and MASH
- CagriSema (Novo Nordisk) — combination of cagrilintide + semaglutide; Phase 3 read-outs 2026
- Pemvidutide (Altimmune) — dual GLP-1/glucagon; clinical development for obesity and MASH
How to choose among FDA-approved weight-loss GLP-1s
- Coverage: What does your insurance cover for weight management?
- Dosing preference: Injection (weekly preferred for adherence) or pill (Foundayo > oral Wegovy on convenience)
- Comorbidities: Heart disease → Wegovy (SELECT CV outcomes data). OSA → Zepbound (OSA approval).
- Cost: Manufacturer programs differ by drug. See GLP-1 cost.
